Diagnosis & Follow-up: For providing TCM consultation, treatment planning, and appointment reminders.

閣下是否同意接收上述市場推廣資訊？ Do you consent to receive the aforementioned marketing information?

閣下是否同意本診所將您的臨床資料作匿名化處理後，用於臨床研究、療效分析及配方優化？

Do you consent to the Clinic using your clinical data in anonymized form for clinical research, treatment response analysis, and formulation optimization?

閣下是否同意本診所在匿名化及不可識別您身份的前提下，使用您的個案資料作健康教育及公開分享（包括網站或社交媒體）？

Do you consent to the Clinic using your case information for health education and public sharing (including website or social media), provided it is anonymized and non-identifiable?

如涉及照片、影片或治療前後對比，本診所會在每次發布前再次徵求確認。
If photos, videos, or before-and-after comparisons are involved, the Clinic will seek confirmation again before each publication.
